Yearly Requirements for Student Athletes

First Time Athletes at Boston College:

A completed physical exam, and immunization records from the primary care physician and a family health history. The physical should take place in the 6 months prior to the beginning of classes. These forms will be sent in a university mailing and returned to Health Services.

A signed copy of the Insurance Information Letter

. This letter should be mailed to Sports Medicine at the following address:

A completed Intercollegiate Insurance Form

. This form should also be sent to the address above. Fax copies do not print legibly and will only be accepted as a temporary form.

A completed Sports Medicine History Form

. This form should be sent to the address above.

Returning Athletes:
